Ambulance Alliance Network Agreement

Purpose: The purpose of this Agreement is to establish the Ambulance Alliance Network, aimed at providing ambulance services to the community either for a fee or free of charge.

Terms and Conditions:

1. 1. Services Provided:
      • The Provider agrees to make their ambulance services available to the community as part of the Ambulance Alliance Network.
      • Services can be provided either for a fee or free of charge, as determined by the Provider.
   2. Ambulance Details:
      • The Provider agrees to provide detailed information about the ambulances they have, including but not limited to:
        • Type and model of the ambulance
        • Equipment available on the ambulance
        • Availability and operating hours
        • Contact information for coordination
        • Drivers registration and training of the driver as appropriate
        • Introduction of the system to the call centers if the have one
   3. Responsibilities of HEARTS:
      • HEARTS will coordinate with the Provider to ensure efficient and effective use of ambulance services.
      • HEARTS will promote the availability of the Provider’s ambulance services to the community.
   4. Payment and Fees:
      • If services are provided for a fee, the Provider will set the rates and handle all billing and payment processes.
      • HEARTS will not be responsible for any payments or fees related to the ambulance services provided by the Provider.
   5. Duration and Termination:
      • This Agreement will remain in effect from the day signed below until terminated by either party with a thirty-day notice day written notice.
      • Either party may terminate this Agreement for any reason, with or without cause.
   6. Miscellaneous:
      • This Agreement constitutes the entire agreement between the parties and supersedes all prior agreements and understandings.
      • Any amendments to this Agreement must be in writing and signed by both parties.
